MEDICAL RELEASE AND WAIVER OF LIABILITY
You must sign this “Release and Waiver Form” to participate in H.O.M.E.
H.O.M.E. is a non-profit, volunteer-based organization that provides a forum for conducting enrichment classes from a Christian worldview for families that home educate their children, regardless of their race, national or ethnic origin, denomination, or educational handicap.
I, my family, and any of my representatives, hereby release H.O.M.E. and Faith Missionary Church and their respective officers, members, employees, and volunteers (referred to collectively here as H.O.M.E.), from any and all liability for damage and injury to my children or myself or to any person or property during the time of my, or my family’s attendance at H.O.M.E. activities whether or not such damages were sustained in connection with any H.O.M.E. activities. I hereby agree to indemnify and hold harmless H.O.M.E. from any claims, including the cost or expenses, arising in connection with injuries or damage sustained by me or my children or attendants. I assume full responsibility for my family’s behavior, for assuring their supervision during H.O.M.E. activities, and for any damage or injury caused by my family’s actions. I willfully and knowingly waive any and all liability claims against H.O.M.E., Faith Missionary Church, their representatives, agents, or any successive entities, which may arise due our family’s participation in any activities coordinated by H.O.M.E. I agree to hold H.O.M.E. and Faith Missionary Church harmless for any and all expenses and/or judgments that may arise, including but not limited to, reasonable attorney fees and court costs. Should any problems resulting in legal action occur, all parties will agree to binding arbitration. I also acknowledge that I am personally responsible for any damage to the facilities caused by my child(ren) or myself, or any injuries sustained by my children or myself.
We, the parents or legal guardians of all the children I am registering and hereby give permission for the above children to participate in H.O.M.E.
We believe that the necessary precautions and plans for the care and supervision of the students during the classes will be taken. Therefore, we will not hold Faith Missionary Church, H.O.M.E., or those supervising the classes responsible for any unavoidable accident. Should an accident occur, we give the sponsor authority to act on our behalf in providing immediate medical care in the nearest medical facility. Prior to care being given, we understand that every reasonable effort will be made to contact us.
TO MEDICAL FACILITY – As the parent(s) or legal guardian(s) of the minor listed above, I hereby authorize the holder of this form to act upon my behalf in the immediate medical care of my minor child(ren) in the event of an emergency situation in which all reasonable effort to reach me has failed.
I further acknowledge and certify that I am the legal guardian or parent of the minors listed above.
H.O.M.E Policies
H.O.M.E. Non-Refund Policy
All requests for refunds must be submitted in writing within the time frame specified. The written request must state why you are interested in a refund. All refunds will be given at the Board’s discretion and per the Non-Refund Policy. Absolutely no refunds will be made if a written request is not received during the Non-Refund Policy period. Please understand that NO refund (full or partial) will be given after one (1) week has passed from the first semester day of co-op. You will have (1) one week from the first day of co-op to decide to discontinue your commitment to co-op. If you choose to drop out any time after the first week, NO refund will be given regardless if you attended or missed the first co-op. No refund will be given after September for non co-op members.
H.O.M.E. Content Policy
Since H.O.M.E. is a Christian organization, we strive to make sure that the atmosphere that we create at H.O.M.E. events is glorifying to God. To make sure that we provide this atmosphere, all items brought to co-op and all other H.O.M.E. events and activities should reflect a Christian or neutral stand point. This includes but is not limited to the following items: backpacks, notebooks, pencils, show and tell items, clothes, jewelry, hats and other such items. The H.O.M.E. Board recognizes that there are a lot of gray area as to what is acceptable and what goes against the Bible. While we don’t desire to offend anyone, we also want to be sensitive to our families and more importantly, to God’s heart. The H.O.M.E. Board has created a policy that items portraying content in the following areas are to be left at home:
witchcraft, Astrology, anything to do with psychic phenomenon, skulls not in relation to the human body, spellcasting, ghosts, or any other content that might be anti-Biblical in content. We appreciate your understanding and cooperation in this matter.
Attendance Policy
If you missed or are late for three or more co-ops in a single semester, your membership application will be on probation for the following semester. If you are on a probationary period and have incurred another 3 tardies/absences, your membership will be up for review by the H.O.M.E Board to determine whether your application will be approved or denied. You will receive an email making you aware if you are in either of the above situations.

1. We will exercise diligence in teaching our children in a responsible way and to use a clearly organized program of education to instruct our children.
2. I understand that I am responsible for the actions and whereabouts of my children at all H.O.M.E. meetings and functions. I agree to be present at the aforementioned activities to provide parent supervision, unless I specifically designate another attending H.O.M.E. parent to be responsible for my children, in which case, written notice is to be submitted to the member in charge of the activity, informing them of the transfer of responsibility.
3. I willfully and knowingly waive any and all personal liability claims against H.O.M.E Faith Missionary Church, their representatives, agents or any successor entities, which may arise due to our family’s participation in any activities coordinated by H.O.M.E. I agree to hold H.O.M.E. harmless for any and all expenses and/or judgments that may arise, including, but not limited to, reasonable attorney fees and court costs.
4. I have read the H.O.M.E. Constitution and understand what is expected of me as a member. I agree to demonstrate commitment to the group by making every effort to support and encourage the accomplishment of its purpose.
H.O.M.E. Student Participation Form
H.O.M.E. is a support group that gives you the opportunity to interact with other home schooling families in co-op, academics, and social events. If you have committed your life to Jesus Christ, you are to grow as He did; in wisdom, stature and in favor with God and man. Our wisdom comes from God and from His Word and He is pleased with us when we love and obey Him. He uses us as His witnesses when our light shines before others that they too might glorify our Father in heaven.
Before beginning this new year as a participant, please read and plan to follow these guidelines:
I agree that I will abide by the guidelines written above. I will, with God’s help, follow the principle set in Matthew 7:12, “So in everything, do to others what you would have them do to you, for this sums up the Law and the prophets.”
Statement of Faith
*If you have a desire to home school and cannot sign this form, we will direct you on how to start your own group or help you find a group in the area.
H.O.M.E. CONSTITUTION
H.O.M.E. (Homes Organized for Meaningful Education) is an association of Christian families who recognize that the education and nurture of their children in all phases of life is the God-given responsibility of the parents. The organization and all of its activities and publications will be consistently and forthrightly Christian.
A. Purpose of H.O.M.E. is:
1. To provide a ministry for mutual support and encouragement between christian families who choose to educate their children in the home.
2. To promote the sharing of information about homeschooling.
3. To organize educational opportunities that would be difficult for individual families to provide for themselves.
B. Membership in H.O.M.E. is open to families who:
1. Agree/support the Statement of Faith, the introductory statement and the purpose of H.O.M.E.
2. Pay an annual participation fee as determined by the board.
3. Parental assistance in group activities is understood as a basic requirement of participation.
4. When a parent is unable to attend an activity and his/her children are in attendance, authorization must be given to another H.O.M.E. parent with written permission (required forms available from H.O.M.E.).
5. Non-members of H.O.M.E. are entitled to attend one co-op as an observer.
6. Are members in good standing of a local, Christian church (definition of “a member in good standing” is one who regularly attends worship and is not under church discipline).
C. Voting Requirements
1. Only members of H.O.M.E. can vote.
2. Each family represents one vote.
D. Officers (Board) of H.O.M.E.
1. Must be a member of H.O.M.E. for 1 year.
2. The board consists of the following officers:
a. H.O.M.E. Director – Group spokesman, oversees all activities.
b. Social Media Coordinator - Oversees all social media.
c. Special Events Director - Oversees and make arrangements for special events.
d. Field Trip Coordinator – Coordinate field trips for group for school year.
e. Co-op Director - Responsible for overseeing the operation of the H.O.M.E. Co-op.

4. Elected at the annual business meeting for a one year term beginning July 1.
5. All decisions of the board must be agreed upon by a simple majority of votes.
E. Financial Matters
1. All H.O.M.E. Board members will be expected to serve without compensation, however, if the budget allows for it, membership fees or portions there of, can be waived as a thank you gift for the time and commitment given. This gift would be voted upon and granted by the previous year’s board, determined by the amount of money that is able to be allocated for this purpose. To be voted upon at the January meeting.
2. The Co-op Director shall receive monies for H.O.M.E. Sources of such monies include participation dues and donations.
3. A financial report shall be presented at the annual business meeting by the Co-op director.
4. No H.O.M.E. monies shall be used to the benefit of, or be distributed to, its members, officers, or other private persons, except that H.O.M.E. shall be authorized and empowered to pay reasonable compensation for services rendered and to make payments and distributions in furtherance of the purposes of H.O.M.E.
5. Upon the dissolution of H.O.M.E., the Board shall, after satisfying all the liabilities of the organization, dispose of all H.O.M.E. assets in a manner consistent with the purposes of H.O.M.E., to such organizations or causes as the Board may determine.
F. H.O.M.E. Activities
1. Co-op Meetings
a. To be held regularly during the school year.
2. Educational Activities
a. To offer educational classes for all ages taught by member parents on a volunteer basis.
b. Classes, workshops and field trips will be organized for participants of H.O.M.E. (contingent upon filled coordinator positions)
c. H.O.M.E. Board must approve all expenses for activities.
3.) Membership Meetings
a. Voting for new board members and changes in the constitution and bylaws will be held once a year either in person or by email.
4. Board Meetings
a. To be held at least twice a year.
G. Amendments
1. All H.O.M.E. members may suggest amendments to be considered by the board.
2. Amendments are proposed to the membership by the Board.
3. Notice of proposed amendments to the constitution should be given no later than two weeks prior to the vote being taken.
4. This constitution can be amended by 2/3 majority of votes.
5. Proxy ballots or email voting will be accepted.
6. All members will be given a two week time frame in which to turn in their votes. This constitution can be amended by 2/3 majority of votes received within the two week time frame.
a. All members who do not turn in their vote within the two week voting period will forfeit their vote for that amendment.

HOME Dress Code Policy
Tops
- Absolutely no cleavage showing
- If you place your closed right hand with your thumb at the top of your STERNUM, which is just below your CLAVICLE, you should not see any skin showing below your hand (your sternum extends all the way up and meets your clavicle - see photo) LOL for all who went to the meeting! You should be able to do this at any time you are wearing your shirt. Many shirts may be fine when you first put them on, but they tend to slide down as you wear them. If in doubt, stick to crew neck shirts.
- No skin showing at waist area (no midriff tops)
Bottoms
- Shorts should be fingertip length (middle finger)
- Please try to have looser fitting pants/shorts.
- No spandex pants – unless they are covered with a shirt that comes down to MID-THIGH or loose fitting shorts
- When sitting in your pants, you should not have a gap between your shirt and pants that shows any part of your waist or bottom. Make sure you have a long enough shirt to cover when you sit down (especially on the floor for co-op lunch). If in doubt, bring a sweatshirt to wrap around your waist.

- I believe the Bible is the inspired and infallible Word of God and constitutes completed and final revelation. The Bible, in its original autograph, is without error in whole and in part; including theological concepts as well as geographical and historical details. (II Tim. 3:16, II Peter 1:21)
- I believe God has existed from all eternity as Father, Son and Holy Spirit. Jesus Christ was God come in human flesh being fully God and fully man except without sin. (Gen. 1:1, John 10:30, John 10:37-38, Matt. 1:23, Hebrews 2:14, John 1:1-3)
- I believe all men are in violation of God’s righteous requirements and His holy character both by nature and sinful act; and are therefore under His wrath and just condemnation. The central purpose of the coming of Jesus Christ was to pay the penalty for man’s sin through His substitutionary death on the cross -- the successful visible, bodily resurrection. (Romans 3:23, 4:24-25, 5:8, Acts 3:15, Psalm 51:5)
- I have accepted salvation through Jesus Christ, which is offered as a gift; free to the sinner.
